Please Note:

  • Current students request transcripts through Naviance, not the form below. 
  • Unofficial transcripts will only be given to the graduate. 
  • Official transcripts will be sent to a University, Recruiter or Employer only. 
  • Transcript requests could take up to 5 business days to complete.

Only former students may use this form to request a transcript. Graduates, please use this fillable transcript request form, fill it out with a copy of your photo ID (Government issued driver’s license or passport). Email  transcript@mtsd.org if you have questions:

Student Records Requests

Only school entities may reach out for student records, upon the transfer of a former MTSD student to their district. Student records can be attained by reaching out to the student’s former building via the building secretary.

Use the links below to find the student’s former school building and click on the link to be redirected to that building’s website for contact information. General contact information is listed at the bottom of each website’s home page.
